What Is Biceps Tendonitis?
The long head of the biceps tendon runs from your upper arm into the shoulder, attaching to the top of the shoulder socket. This tendon helps stabilize the shoulder and assists with lifting, reaching, and overhead movements.
Biceps tendonitis occurs when the tendon becomes irritated or overloaded, often due to repetitive overhead activities, sports, weightlifting, or age-related wear. It frequently occurs alongside other shoulder conditions such as rotator cuff tendonitis, shoulder impingement, or SLAP tears.
Fortunately, most cases of biceps tendonitis respond very well to conservative treatment with physical therapy.
Common Symptoms
You may have biceps tendonitis if you experience:
✔ Pain in the front of the shoulder
✔ Tenderness over the biceps tendon
✔ Pain with lifting or reaching overhead
✔ Pain during pushing or pulling activities
✔ Pain when throwing or serving in sports
✔ Discomfort while lifting weights, especially curls or overhead presses
✔ Shoulder weakness or fatigue
✔ Pain when sleeping on the affected shoulder
Common Causes
Biceps tendonitis commonly develops from:
Repetitive overhead activities
Weightlifting
Throwing sports (baseball, tennis, volleyball)
Shoulder impingement
Rotator cuff weakness
Poor shoulder mechanics
Repetitive work activities
Age-related tendon degeneration
During your evaluation, we'll determine why the tendon became irritated and address the underlying movement impairments contributing to your symptoms.

Frequently Asked Questions
Will biceps tendonitis heal without surgery?
Yes. The vast majority of people with biceps tendonitis recover successfully with conservative treatment, including physical therapy, activity modification, and a progressive strengthening program.
Do I need an MRI?
Not usually. Biceps tendonitis can often be diagnosed with a thorough physical examination. Imaging may be recommended if a rotator cuff tear, labral tear, or another shoulder injury is suspected.
Should I stop exercising?
Not necessarily. In many cases, you can continue exercising with temporary modifications to avoid aggravating the tendon while it heals. Your physical therapist will guide you on which exercises are safe.
How long does recovery take?
Many people notice significant improvement within 6 to 12 weeks, although recovery depends on the severity of the condition, how long symptoms have been present, and consistency with treatment.
